Professional Referral Form

This referral form is for professionals to refer medium risk service users. Please ensure that all High Risk referrals continue to go through the MARAC process referral system.

Any standard risk referrals continue to go through Norfolk and Suffolk Victim Care. Please be advised that NIDAS is unable to find safe accommodation spaces for clients.

Risk Indicators

High: There are identifiable indicators of risk of serious harm. The potential event could happen at any time and the impact would be serious. (or 14+ ticks)

Medium: There are identifiable indicators of risk of serious harm. The perpetrator has the potential to cause serious harm but is unlikely to do so unless there is a change in circumstances (ie, drug/alcohol misuse). (or 10-13 ticks)

Standard: Current evidence does not indicate likelihood of serious harm. (or 0-9 ticks)
